Introduction

VHIS Certified Plans may suit people seeking regulated individual indemnity hospital cover and access to the VHIS tax deduction, while international medical insurance may suit people seeking broader geographical cover. Some clients may combine the two, but the policies do not automatically operate as seamless base and top-up layers; the terms, benefits and coordination of claims must be compared.

This guide compares the two on what they cover, what they cost, and how portable they are, so you can work out which fits your situation.

Key takeaways

  • VHIS premiums may be lower for some plan designs, and qualifying premiums under Certified Plans are tax-deductible subject to the statutory rules
  • International plans may offer broader geographical cover and higher annual limits, subject to the selected plan and relocation rules
  • The VHIS Standard Plan benefit schedule has an annual benefit limit of HK$420,000
  • International-plan annual limits vary; published Bupa Global Hong Kong plans currently range from HK$35 million to HK$78 million
  • Some clients combine a VHIS Certified Plan with international medical insurance, subject to the terms of both policies

What is VHIS?

VHIS is the Voluntary Health Insurance Scheme, a government framework launched in 2019 that sets minimum standards for certified private medical plans in Hong Kong.

Every certified plan must meet a fixed set of protections, which makes VHIS predictable.

  • Guaranteed renewal up to age 100
  • No lifetime benefit limit on certified plans
  • Minimum phased cover for unknown pre-existing conditions: 0% reimbursement in policy year 1, 25% in year 2, 50% in year 3 and 100% from year 4, subject to policy terms and benefit limits
  • A deduction for qualifying VHIS premiums, capped at HK$8,000 per insured person per year; this is a deduction from assessable income, not a direct HK$8,000 reduction in tax payable

VHIS has two types of Certified Plans. Standard Plans follow standardised minimum benefits, subject to permitted variations, and include an annual benefit limit of HK$420,000. Flexi Plans must provide protection equivalent to the Standard Plan plus enhanced protection, but their benefit limits, geographical coverage and additional benefits vary by product.

What is international health insurance?

International private medical insurance, often called IPMI, may provide regional or worldwide cover. Continued cover after relocation depends on the insurer, destination, eligibility rules and policy terms; portability should not be stated as automatic.

It is built for higher limits and broader treatment than a local plan.

  • Worldwide cover, including or excluding the USA
  • Annual limits vary by plan; for example, Bupa Global’s published Hong Kong plans currently show limits from HK$35 million to HK$78 million
  • Portability may be available, but depends on the insurer, destination and policy terms
  • Cancer, evacuation and outpatient benefits vary materially by insurer, plan tier and optional benefits

International cover may cost more than local cover because of its geographical scope and benefit design. Premiums vary by age, area of cover, deductible, plan tier, optional benefits, medical history and insurer.

VHIS vs international health insurance compared

The two products solve different problems. The table below sets them side by side on the points that decide most cases.

  VHIS  International
Where you are covered  Primarily Hong Kong focused; some Flexi Plans provide regional or worldwide benefits  Regional or worldwide, depending on the selected plan
Portability  Policy ownership is not tied to an employer, but overseas treatment and continued cover after relocation depend on the plan terms  Moves with you
Annual limit  HK$420,000 under the Standard Plan; Flexi Plan limits vary and can be substantially higher  Varies by plan; published examples extend into tens of millions of Hong Kong dollars
Tax deduction  Qualifying premiums deductible up to HK$8,000 per insured person per year  No
Typical cost, age 35  Official examples at age 35: Bupa MyFlexi Standard HK$4,044; Bupa MyFlexi Advance HK$8,548 annually  Indicative broker range for ages 35–44: HK$22,000–HK$65,000 annually for a full international plan
Best for  People whose expected treatment needs align with the selected Certified Plan  People seeking regional or worldwide cover, subject to relocation and eligibility terms

Premium sources: official VHIS premium schedules for Bupa MyFlexi Standard and Advance show annual premiums of HK$4,044 and HK$8,548 respectively at age 35. A Hong Kong broker’s 2026 market guide gives an indicative HK$22,000–HK$65,000 range for full international plans at ages 35–44. These are not like-for-like quotations. Actual premiums depend on age, health, geographical area, deductible, benefits, insurer and underwriting.

Which one is cheaper?

Premiums should be compared on a like-for-like basis. However, local-only plans cost less than international plans, especially if you compare a VHIS Standard Plans vs International Plans. Official VHIS schedules illustrate the variation: at age 35, Bupa MyFlexi Standard is HK$4,044 a year and Bupa MyFlexi Advance is HK$8,548 a year, before the Insurance Authority levy. A 2026 Hong Kong broker market guide gives an indicative range of HK$22,000 to HK$65,000 a year for a full international plan for ages 35–44.

  • Standard Plans generally provide the minimum standardised VHIS benefits, but the cheapest premium depends on age, insurer and current premium schedule
  • Flexi Plans add enhanced protection, but both the benefits and premium differences vary materially by product
  • International cover may cost more, but the premium depends on geographical area, deductible, plan tier and optional benefits

Qualifying VHIS premiums may be deducted from assessable income up to HK$8,000 per insured person per year. The tax saved is not automatically HK$8,000; it depends on the qualifying premium paid and the taxpayer’s circumstances. Premiums attributable solely to a non-VHIS international plan do not qualify.

Do I need both VHIS and international insurance?

Some clients may choose both because the products can address different needs. A VHIS Certified Plan provides scheme protections including guaranteed renewal up to age 100 and access to the tax deduction, while an international plan may add broader geographical cover and higher limits. Whether they work together depends on both policies’ terms and claims coordination.

  • A VHIS Certified Plan for qualifying premiums and scheme protections
  • International cover for eligible treatment within the selected geographical area
  • Combined cover may broaden protection, but gaps, overlap and coordination rules must be checked

This combination may suit clients whose current and expected geographical needs are not met by one policy alone. Suitability depends on budget, health, destination, benefit design and the interaction between the policies.

Which insurers offer VHIS and international plans?

Lifestyle Insurance works with insurers offering VHIS Certified Plans, international medical insurance, or both. The appropriate option depends on budget, health history, expected treatment locations, benefit requirements and the terms governing how multiple policies respond to a claim.

Insurer  VHIS  International
Bupa Hong Kong  Yes  Via Bupa Global
Cigna   Yes  Yes
AXA Hong Kong  Yes  Yes
Allianz Care  No  Yes
APRIL International  No  Yes

Lifestyle Insurance compares VHIS and international medical insurance options from its insurer panel and helps clients assess which structure is suited to their circumstances. Confirm the exact licensed entity name and licence particulars against the Insurance Authority register before publication.

The bottom line

VHIS and international medical insurance are different product categories rather than automatic layers. VHIS provides scheme-standard protections and an income tax deduction for qualifying premiums, while international plans may provide broader geographical cover and higher limits. The right choice depends on expected treatment locations, budget, benefits, health, relocation plans and policy terms.

FAQs

Is VHIS better than international health insurance?

Neither is inherently better. Qualifying VHIS premiums are tax-deductible and Certified Plans include scheme protections; international plans may provide broader geographical cover and higher limits. Premiums and portability vary by product.

Can I claim tax on international health insurance in Hong Kong?

Only qualifying premiums attributable to a VHIS Certified Plan are deductible, capped at HK$8,000 per insured person per year. A premium attributable solely to a non-VHIS international plan does not qualify.

Does VHIS cover me outside Hong Kong?

The Standard Plan is primarily designed around Hong Kong treatment. Some Flexi Plans provide regional or worldwide benefits, but geographical limits and emergency-only provisions vary. International plans may provide broader overseas cover, subject to the selected area, exclusions and policy terms.
